Review how the documented concerns relate to classroom functioning, independent work, testing conditions, transitions, task completion, and other relevant educational demands.

Recommendations and Support Plan
Identify the recommendations most relevant to the school team’s current questions.
Recommendations provide professionally grounded considerations for discussion. They do not automatically determine school services, accommodations, or implementation.

Monitor and Continue Review
When clarification is needed, trace the interpretation back to the relevant parent or caregiver and school information, clinical history, structured assessment findings, and documented information limitations.

and boundaries of individual evidence layers.
No individual score, chart, or task result should be treated as a conclusion on its own.
Start with the overall interpretation and educational impact. Use the supporting evidence and limitations to understand how the conclusions were reached and where additional context may still be needed.
Reading the Supporting Evidence
Read the Record as a Whole, Not as Isolated Scores
The supporting sections help school professionals understand how the clinician reached the final interpretation. They should be reviewed together, with attention to source, context, functional impact, and documented limitations.
Focus on Functional Impact
Consider what the findings may mean for the student’s participation, task initiation, work completion, organization, regulation, independent work, transitions, and ability to demonstrate knowledge.
Compare Sources Without Ranking Them
Parent, caregiver, educator, and other observations reflect different settings, demands, and opportunities for observation. Differences between sources may provide meaningful context and do not by themselves establish that one source is more reliable than another.
Interpret Assessment Findings as Supporting Evidence
Structured task performance may add context to patterns observed during the assessment. It should not be treated as a standalone conclusion.
Review Context and Limitations
Consider missing information and relevant factors identified in the report, including language, learning history, sleep, medication, testing conditions, and other circumstances that may affect interpretation.
A score, percentile, chart, color band, or task result does not independently establish a diagnosis, educational eligibility, or the need for a particular support.

Which concerns, strengths, and patterns are reflected in classroom observations, school records, academic performance, and available support data?
Where Do the Sources 
Converge or Differ?
Which observations are consistent across home and school, and which differences may reflect changes in setting, expectations, or task demands?
What Is the Functional Impact?
How do the documented concerns affect participation, task initiation, work completion, organization, transitions, independent work, or the student’s ability to demonstrate knowledge?
What Has Already Been Tried?
Which supports or interventions have been implemented, how consistently were they used, and how did the student respond?
What Context or Information Is Missing?
Are there limitations, unanswered questions, or additional school-based information the team should consider as part of its review?
Which Recommendations Are Relevant to the Current Discussion?
Which recommendations relate directly to the team’s current questions, and how might they be considered alongside school data and professional judgment?
